Chapter 3 - Shadows Over the Estate

The golden autumn weeks turned into crisp November air, and a fragile, beautiful rhythm began to take root inside the Caruso estate.

For the first time in four years, the cold, silent mansion felt alive.

Sophia did not abandon her calling. Though she took a temporary leave of absence from full-time surgical shifts at St. Agnes, Marco arranged for a state-of-the-art pediatric clinic to be established in a renovated wing of the estate, where she could treat underprivileged children from the city, funded entirely by the Caruso Foundation.

And slow, miraculous changes began to happen with Tommy.

At first, the boy remained close to Sophia, trailing behind her like a shadow whenever she walked through the sprawling estate. But slowly, under Marco’s patient, surprisingly gentle guidance, the ice between father and son began to melt.

Marco did not push. He did not demand that the boy call him "Papa." Instead, he spent hours sitting on the carpeted floor of the library, teaching Tommy how to play chess, building complex wooden models of ships, and reading him old Italian fairytales before bed.

One afternoon, Sophia stood near the tall French doors of the conservatory, holding a warm cup of tea, watching the two of them in the garden outside.

The first light snowfall of the season was dusting the green lawn. Tommy, bundled in a heavy red winter coat, was running through the grass, chasing a golden retriever puppy Marco had bought for him two weeks prior. Marco walked beside him, his heavy wool coat open, his face relaxed, laughing openly as the boy tripped over his own boots and tumbled into a snowbank.

Tommy sat up, covered in white flakes, and let out a bright, ringing laugh—a sound so pure, so unburdened by horror, that it filled the entire courtyard.

Marco dropped to his knees in the snow, scooping the boy up into his arms, tossing him gently into the air before pulling him tightly against his chest. Tommy didn't flinch. He didn't pull away. He wrapped his small arms around Marco’s neck, burying his face in his father’s shoulder.

Sophia felt a soft, warm tear slip down her cheek.

"He smiles more now," a deep voice spoke beside her.

Sophia turned to see Gianni standing near the glass doors, holding a clipboard. The intimidating underboss was looking out at the garden with a look of profound, quiet gratitude.

"Four years ago, Dottoressa," Gianni said softly, "I thought I’d lost my friend forever. Don Marco was a dead man who just hadn't stopped breathing yet. You brought him back. You brought both of them back."

Sophia looked down at her tea, a faint blush warming her cheeks. "He did it himself, Gianni. He loved his son enough to wait."

"He loves his son, yes," Gianni agreed, his sharp eyes shifting to look directly at Sophia’s face. "But he looks at you the same way he looks at the boy, Dottoressa. Like you are the air he’s been drowning for without realizing it."

Sophia’s heart gave a sudden, unexpected flutter.

Over the past two months, her feelings toward Marco Caruso had undergone a terrifying, irresistible evolution. She had watched the ruthless syndicate Don transform into a fiercely protective, devoted father. She had seen the quiet nobility in his actions, the absolute honor with which he treated his household, and the subtle, electric warmth in his eyes whenever he looked at her across the dinner table.

He had respected every boundary she had set. He had never forced himself into her space, never used his power to intimidate her, and treated her opinions regarding Tommy’s raising with absolute authority.

She was falling in love with a mafia boss. The realization frightened her more than any gun ever could.

That evening, after Tommy had fallen fast asleep in his bedroom—clutching his wooden toy ship and his silver St. Christopher medal—Sophia walked down to the estate’s private wine cellar to retrieve a bottle for dinner.

The cellar was a subterranean sanctuary of cool stone, vaulted ceilings, and thousands of vintage bottles stored in dark wooden racks.

As she reached for a bottle of Tuscan red, she heard a heavy, rhythmic step behind her.

She turned to see Marco stepping down the stone stairs. He had discarded his suit coat, his white shirt sleeves rolled up to his forearms, revealing the dark, intricate ink covering his skin.

"Rosa said you were down here," Marco said softly, his deep voice echoing pleasantly against the stone walls.

"I was just picking something for dinner," Sophia said, offering him a faint smile. "Tommy went to sleep early. He was exhausted from his snow fight."

Marco walked over, stopping two feet away from her. The cool air of the cellar was filled with his scent—expensive cedar, rain, and faint tobacco.

"Thank you, Sophia," Marco said softly.

Sophia tilted her head. "For what?"

"For giving him back his childhood," Marco said, his gray eyes locking onto hers with a fierce, burning intensity. "For allowing me to be his father. Every night when I watch him sleep, I know that without you, none of this would exist. I would still be a ghost in an empty house."

"Marco..." Sophia whispered, her breath catching in her throat as he took a step closer, closing the remaining distance between them.

"I know who I am, Sophia," Marco murmured, his long, warm fingers reaching out to gently touch the side of her face, his thumb brushing over her cheekbone with unbelievable tenderness. "I know my hands are stained with blood. I know my world is dark and dangerous. I tried to stay away from you. I tried to keep my feelings buried because you deserve a clean life, a normal man."

Sophia reached up, her small hand resting over his large, scarred hand against her cheek. "I don't want a normal man, Marco. I want a man who fights for the people he loves. I want you."

Marco’s dark eyes flashed with an overwhelming wave of passion. He didn't hesitate. He leaned down, his lips meeting hers in a deep, slow, breathtaking kiss.

It was a kiss filled with four years of buried grief, unspoken gratitude, and a fierce, unyielding promise of protection. Sophia wrapped her arms around his neck, pulling him closer, melting against his broad, muscular chest as the cold stone cellar dissolved around them.

When he finally pulled back, his forehead rested against hers, both of them breathing heavily in the quiet room.

"I will protect you with my life, Sophia," Marco swore softly against her lips. "You and Matteo. Always."

Before she could answer, a loud, frantic alarm shattered the peace of the mansion above.

BEEP! BEEP! BEEP!

The heavy red emergency lights in the cellar began to flash, casting a bloody glare across the stone walls.

Marco’s entire posture shifted instantly. The tender lover vanished, replaced in a split second by the lethal, terrifying Don. He drew a heavy pistol from his waistband, pulling Sophia behind his broad frame.

Gianni ran down the stone steps, his face pale, his weapon drawn.

"Don Marco! We have a breach at the west gate!" Gianni yelled frantically. "It’s a hit squad! They bypassed the outer sensors using military-grade jammers!"

Marco’s eyes narrowed into terrifying slits of ice. "Who is it?"

"It's the remnants of the Moretti clan," Gianni spat, his jaw clenching. "Led by Vincent Moretti—the brother of the man you killed during the Harbor War. They know Matteo is alive, Marco! They came to finish what they started four years ago!"

Sophia let out a strangled gasp, her hands gripping Marco’s shirt in terror. "Tommy!"

Marco turned to her, catching her face in both his hands, forcing her to look into his dark, deadly eyes.

"Listen to me, Sophia," Marco commanded, his voice an absolute iron decree. "Take Gianni. Go through the secret passage behind the wine rack. It leads straight to the underground panic room beneath the east wing. Lock the steel door from the inside and do not open it for anyone except me."

"Marco, no!" Sophia sobbed, clutching his arms. "There are too many of them! Come with us!"

"I am not running in my own home, Sophia," Marco whispered, pressing a hard, fierce kiss to her lips. "I spent four years running from my past. Tonight, I end it forever. Go to our son. I will come back to you."

He released her, turned to Gianni, and barked a single order: "Get them to the bunker!"

Before Sophia could protest, Gianni grabbed her by the arm, pulling her toward the back of the wine cellar, where a heavy stone panel slid open into a dark, concrete tunnel.

Outside, the thunderous roar of automatic gunfire tore through the quiet autumn night.
